Our Fund helps children affected by cancer and leukemia. If your child is affected by this tragedy, we will try to do our best. But in order to be able to transfer your request to involved and sympathetic people we need a detailed letter about your situation. E.g. if the issue is the cost of treatment of your child which you cannot afford, the following documents are necessary:
- an agreement with a hospital about a surgery;
- a bill issued by the hospital including banking information;
- an excerpt from the medical history of the child;
- income certificates for the parents or a decision by local social security authorities about financial situation of the family;
- the birth certificate of the child;
- your postal address, contact phone numbers;
- a photo of the child in color;
- Individual Tax-Payer number (IPN) of the person writing the letter;
- number of state insurance pension certificate of the participant.
You should write the letter in person and make sure that you include the following lines: “I do not object to mass-media coverage (including the Internet) of all the documents, medical excerpts and photos provided by me”
